One of the most heartbreaking things that can happen to a family is to learn that their child is seriously ill and needs lifesaving medical care. Suddenly, life changes overnight and the world is filled with uncertainty and fear.

For many of these families, the best medical treatment available is often an ocean away. In addition, families face the financial demands of mounting medical expenses, job insecurity, separation from family and friends, and a healing journey that can take weeks, months, or longer.

For over 38 years, Ronald McDonald House Charities Hawaii has welcomed nearly 15,000 families. They provide a home-away-from-home, healthy meals, transportation, and a loving community of compassion where families can heal and grow stronger together completely free of charge.

In consideration of my (I, my or myself) and/or on behalf of my child/wards (each a Ward) participation in the Ronald McDonald House Charities of Hawaii (RMHC-HI) Event (the Event) as a participant, volunteer (or other staff member) (any of the foregoing, a Participant), I, the Participant, and all participants for which I register, acknowledge, accept and agree the following:

I, on behalf of myself and Ward are in good physical condition. I know that this Event is a potentially hazardous activity and I hereby assume full & complete responsibility for, and for the risk of, any injury or accident that may occur during my participation in this Event or while on the premises of this Event.

To the fullest extent permissible by applicable law, I on behalf of myself and/or my Ward (if applicable), and our respective heirs, assigns, spouses, partners, personal representatives and/or next of kin, forever WAIVE, RELEASE, DISCHARGE, COVENANT NOT TO SUE and hold harmless RMHC-HI, Ala Moana Center (Brookfield Properties) and its directors, officers and agents, including all volunteers and medical volunteers providing services for the Event, from all liability for personal injury, death, property damage and other loss or damage arising from or related to my own actions, the actions of RMHC-HIs clients, volunteers or RMHC-HI.

Photographic Release

I hereby authorize Ronald McDonald House Charities of Hawaii (RMHC-HI) to use my photographs, video, audio and/or story in all materials related to media coverage and RMHC-HI publications.

I agree that electronic submission of this registration constitutes agreement to all the terms of this Release and Waiver statement.

By agreeing to continue, I acknowledge on behalf of myself and/or my Ward having read and fully understand all terms of this Release and Waiver and I hereby agree to the waiver and that I am 18 or older, or that I have the authority to register these participants and agree to the waiver for them. I HAVE READ AND FULLY UNDERSTAND THIS WAIVER. I UNDERSTAND THAT I HAVE GIVEN UP, ON BEHALF OF MYSELF AND MY WARD, SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY.
